Investigação de fatores genéticos na patogênese do pênfigo foliáceo endêmico : análise de associação com genes de citocinas e microssatélites do complexo principal de histocompatibilidade

Resumo: O pênfigo foliáceo endêmico (PFE) é uma doença auto-imune de etiologia complexa, caracterizada pela presença de auto-anticorpos cujo alvo é a desmogleína 1 (dsg-1), os quais induzem a perda de adesão entre os queratinócitos (acantólise). O mecanismo subjacente ao processo acantolítico não é compreendido. A participação de mecanismos celulares não está elucidada, embora células T anti-dsg-1 tenham sido encontradas em sangue periférico de pacientes com PFE. As citocinas que são expressas pelas células T CD4+ presentes no sangue e nas lesões epidérmicas mostram predominância de perfil tipo 2. Características epidemiológicas do PFE sugerem que a exposição a fatores ambientais desencadeie as manifestações clínicas da doença em indivíduos geneticamente suscetíveis. Certas variantes dos genes HLA-DRB1, DQA1 e DQBJ são os únicos fatores genéticos já estabelecidos como moduladores da suscetibilidade e proteção ao PFE. Este estudo teve como objetivo pesquisar outros fatores genéticos subjacentes à patogênese do PFE. Foram investigadas outras regiões do Complexo Principal de Histocompatibilidade (CPH) que pudessem conter genes, além dos genes HLA, envolvidos na fisiopatologia desta doença; foi pesquisada também a influência das variantes de genes das citocinas, uma vez que seus polimorfismos podem afetar o nível de produção destas moléculas imunomoduladoras. A amostra incluiu 168 pacientes com PFE e 189 controles, sendo constituída em sua maioria de caucasóides e negróides, e em menor proporção de índios e mestiços de índios com caucasóides. A investigação de genes de suscetibilidade no CPH foi baseada em um estudo de associação caso-controle, utilizando microssatélites. Os alelos foram tipificados pela reação em cadeia da polimerase (PCR) utilizando oligonucleotídeos iniciadores loco-específicos, marcados com fluoróforos. Os produtos da PCR foram diferenciados por tamanho em analisador de DNA. Nenhuma associação significativa foi encontrada entre o PFE e as variantes dos microssatélites D6S265, D6S273, D6S276, D6S291, M1B, MOGCA, RING3CA e TAP1CA, resultado esse que permite excluir como candidatos os genes próximos a esses marcadores. Com respeito ao microssatélite G51152, centromérico ao loco HLA-DQB1, foram identificadas associações negativas com os alelos *215 (OR = 0,34), *217 (OR - 0,27) e *223 (OR = 0,39) e associação positiva com o alelo *227 (OR = 6,00). Este resultado confirma associações entre o PFE e variantes das sub-regiões DR e DQ, reveladas em estudos anteriores. O efeito da variabilidade dos genes das citocinas foi investigado por um estudo de associação caso-controle, e as variantes, tipificadas por PCR-RFLP. Nenhuma associação foi observada com as variantes de IL1A, IL1B, IL1RN, IL4R e IL10. Nos negróides, houve uma associação negativa fraca com o haplótipo -1082_G -592 C do gene IL10 (OR = 0,49). Em relação ao polimorfismo -590 do gene IL4, foi encontrada uma associação positiva com o genótipo T/T (OR = 2,71) e outra negativa com a variante C (OR = 0,37). Embora os resultados indiquem uma influência das variantes de IL4 -590 na patogênese do PFE, sugerimos que o significado destas associações fracas seja esclarecido em amostra maior. Associações com as variantes de IL6 -174 sugerem um efeito protetor do genótipo C/C (OR = 0,13), enquanto o alelo G parece ser um fator de suscetibilidade (OR = 7,66) ao PFE. A nossa hipótese é que o aumento da expressão da IL-6, influenciado pelo alelo -174 G, favoreça o espalhamento de epítopos que resulta na produção dos auto-anticorpos patogênicos, além de exacerbar as lesões epidérmicas pela indução de mediadores inflamatórios e de contribuir na diferenciação e manutenção dos clones T e B auto-reativos específicos para a dsg-1. Novas mutações de ponto foram identificadas no gene TNF, nas posições -1491, -1391, -1226 e -1380, pelo sequenciamento do DNA de 38 células de referência dos Workshops Internacionais de Histocompatibilidade. Estas novas variantes não foram investigadas para possíveis associações com PFE devido às suas freqüências muito baixas. Abstract: Endemic pemphigus foliaceus (EPF) is an autoimmune disease of complex etiology characterized by the presence of antibodies against desmoglein 1 (dsg-1). The presence of these autoantibodies leads to the loss of adhesion among keratinocytes (acantholysis). The mechanism underlying the acantholytic process has not been understood, yet. Although autoreactive T cells specific to dsg-1 have been found in peripheral blood of patients with EPF, the role played by cellular mediated responses has not been elucidated. CD4+ T cells from the peripheral blood as well as from the epidermal lesions express predominantly a type 2 cytokine profile. Epidemiological features suggest that exposure to environmental antigens triggers the disease in genetically susceptible individuals. Certain variants of HLA-DRB1, DQA1 and DQB1 genes have been the only genetic factors found to modulate susceptibility and resistance to EPF, so far. The aims of this study were to identify other genetic factors underlying the pathogenesis of EPF. Other regions of the Major Histocompatibility Complex (MHC) were investigated for the presence of genes, other than HLA genes, which could be involved in the pathophysiology of EPF. The influence of cytokine gene variants was also investigated due to the fact that their polymorphisms may affect the levels of production of these immunomodulatory molecules. The sample included 168 patients with EPF and 189 controls and was comprised mostly of Caucasoids and Mulattos with a small proportion of Amerindians and Mestizos (Amerindian x Caucasoid). The approach taken for investigating MHC susceptibility genes was an association study in a case-control design using microsatellites. Alleles were typed by polymerase chain reaction (PCR) utilizing locus-specific fluorescent-labeled primers. Amplified products were distinguished by size using a DNA analyzer. No significant associations were found between EPF and allelic variants from microsatellites D6S265, D6S273, D6S276, D6S291, MIB, MOGCA, RING3CA and TAP1CA. This result allows the exclusion of the genes located near to these markers as candidates. With respect to microsatellite G51152, centromeric to HLA-DQB1 locus, negative associations were found with alleles *215 (OR = 0.34), *217 (OR = 0.27) and *223 (OR = 0.39) in addition to a positive association with allele *227 (OR = 6.00). These results confirm associations of EPF with variants of the DR and DQ sub-regions that have been revealed in previous studies. The effect of cytokine genes variability was also investigated by a case-control association study and the alleles were typed by PCR-RFLP. No associations were found with variants of ILIA, IL1B, IL1RN, IL4R and IL10. In Mulattos, there was a negative weak association with haplotype -1082 G -592 C (OR = 0.49) of the IL10 gene. With regard to the polymorphism -590 of the IL4 gene, a positive association with T/T genotype (OR = 2.71) and a negative association with the C variant (OR = 0.37) were found. Although these results indicate an influence of IL4 -590 variants in the pathogenesis of EPF, we suggest that the meaning of these weak associations be elucidated in a larger sample. Associations with the IL6 -174 variants suggest that the C/C genotype has a protective effect (OR = 0.13) while the G allele seems to confer susceptibility (OR = 7.66) to EPF. We hypothesize that over expression of IL-6, influenced by allele -174 G, favors epitope spreading that leads to production of pathogenic autoantibodies. Furthermore, it causes exacerbation of epidermal lesions by inducing other inflammatory mediators and contributes to differentiation and sustaining of the growth of autoreactive T and B cell clones specific to dsg-1. Finally, new point mutations were found in the TNF gene at positions -1491, -1391, -1226 and -1380 by sequencing the DNA of 38 International Histocompatibility Workshop reference cells. Due to the low frequency of the new variants, they were not investigated for possible associations with EPF.
