Impacto da consulta farmacêutica sobre o número de atendimentos a pacientes com asma em prontos atendimentos do município de Porto Alegre

Resumo: A asma é uma doença inflamatória crônica das vias aéreas, cujo tratamento se dá, essencialmente, através do uso de dispositivos inalatórios. A baixa adesão ao tratamento e a falta de conhecimento dos pacientes no uso correto destes dispositivos são umas das principais barreiras para o seu controle adequado. Neste contexto, o farmacêutico tem papel fundamental na educação e orientação dos pacientes para o alcance dos objetivos terapêuticos e melhoria da qualidade de vida. O presente estudo objetivou avaliar o impacto da consulta farmacêutica sobre o número de atendimentos a pacientes com asma em prontos atendimentos (PAs) de Porto Alegre, RS, Brasil. Trata-se de estudo observacional descritivo retrospectivo. Os dados foram coletados por meio de avaliação retrospectiva de prontuários, instrumento de coleta de dados e sistema informatizado próprio. O grau de controle da asma foi avaliado utilizando o questionário Global Initiative for Asthma (GINA). Foram incluídos pacientes com diagnóstico de asma que passaram por atendimento nos PAs municipais nos anos de 2018 e 2019, com prescrição de medicamentos inalatórios da Relação Municipal de Medicamentos Essenciais de Porto Alegre (REMUME-POA) e que passaram por consulta farmacêutica. As análises estatísticas foram realizadas utilizando o programa SPSS versão 22 fixando o nível de significância em 0,05. Dos 129 pacientes que compuseram a amostra final, observou-se predominância de indivíduos do sexo feminino (61,20%) com mediana de idade de cinco anos (IIQ = 20). Conforme avaliação farmacêutica, 51,9% dos pacientes caracterizavam-se como não controlados e apenas 3,9% como bem controlados. Cerca de metade da população (51,9%) possuía prescrição exclusivamente de medicamento de alívio, sem prescrição de corticoide inalatório (CI), enquanto mais da metade (55%) não possuía prescrição de regime posológico de crise. Teste pareado mostrou redução estatisticamente significativa no número de atendimentos nos PAs municipais após a consulta farmacêutica em comparação ao número de atendimentos nesses serviços antes da consulta farmacêutica (z = -7,838, p < 0,001). Os principais erros observados na população do estudo quanto ao uso do dispositivo inalatório foram: velocidade de inspiração rápida (47,3%), intervalo entre jatos menor que 60 segundos (47,3%) e manutenção da Capacidade Pulmonar Total (CPT) menor que 10 segundos (28,7%). Os fatores que estiveram associados à redução significativa no número de atendimentos após a consulta farmacêutica foram: pacientes do sexo feminino (ß= -0.711, p =0.005), tempo de uso do dispositivo entre 1-3 anos (ß= -0.916, p =0.036), intervalo entre jatos menor de 60 segundos (ß= -0.918, p =0.003), não completar a inspiração antes do acionamento (ß= -2.229, p <0.001), não inspirar (ß= -2.608, p <0.001), inspirar antes e expirar depois profundamente (ß= -0.901 [95% CI -1.466; -0.335], p =0.002) e nenhum erro de inspiração registrado (ß= -1.559, p <0.001). A análise dos dados demonstrou que há impacto positivo significativo da consulta farmacêutica sobre o número de atendimentos a pacientes com asma nos PAs de Porto Alegre, RS. Evidenciou-se alto percentual de pacientes utilizando exclusivamente salbutamol, sem prescrição de CI, e de pacientes sem prescrição de regime posológico de crise, indicando necessidade de capacitação dos prescritores. Abstract: Asthma is a chronic inflammatory disease of the airways, whose treatment is essentially through the use of inhalation devices. The low adherence to treatment and the lack of knowledge of the correct use of these devices are one of the main barriers to their adequate control. In this context, pharmacists have a fundamental role in educating and guiding patients to reach therapeutic goals and to improve quality of life. The present study aimed to evaluate the impact of pharmaceutical consultation on the number of visits of asthma patients to emergency care units of Porto Alegre, RS, Brazil. This is an observational, descriptive, retrospective study. Data were collected through retrospective assessment of medical records, data collection instrument and a computerized system used by Porto Alegre Health Department. The degree of asthma control was assessed using the Global Initiative for Asthma (GINA) questionnaire. Patients diagnosed with asthma who underwent care in municipal emergency care units in 2018 and 2019, with prescription of inhaled medicines from the Municipal Relation of Essential Medicines of Porto Alegre (REMUME-POA) and who underwent pharmaceutical consultation were included. Statistical analyzes were performed using the SPSS version 22 program, setting the significance level at 0.05. Of the 129 patients who made up the final sample, there was a predominance of female individuals (61.20%) with a median age of five years (IQR = 20). According to pharmaceutical evaluation, 51.9% of the patients were characterized as uncontrolled and only 3.9% as well controlled. About half of the population (51.9%) had a prescription for relief medication, without prescription of inhaled corticosteroids (IC), while more than half (55%) had no prescription for a crisis dosage regimen. Statistical analysis showed a statistically significant reduction in the number of visits to emergency care units after the pharmaceutical consultation compared to the number of visits to these services before the pharmaceutical consultation (z = -7.838, p < 0.001). The main errors observed in the study population regarding the use of the inhalation device were: rapid inspiration speed (47.3%), interval between jets less than 60 seconds (47.3%) and maintenance of a Total Lung Capacity (TLC) lower than 10 seconds (28.7%). The factors that were associated with the significant reduction in the number of visits after the pharmaceutical consultation were: female patients (ß= -0.711, p =0.005), time of device use between 1-3 years (ß= -0.916, p =0.036), interval between jets of less than 60 seconds (ß= -0.918, p =0.003), do not complete inspiration before triggering (ß= -2.229, p <0.001), do not inhale (ß= -2.608, p <0.001), inhale before and exhale deeply (ß= -0.901 [95% CI -1.466; -0.335], p =0.002) and no inspiration errors recorded (ß= -1.559, p <0.001). Data analysis showed that there is a significant positive impact of the pharmaceutical consultation on the number of visits of asthma patients to emergency care units of Porto Alegre, RS. There was a high percentage of patients using exclusively albuterol, without prescription of IC, and of patients without prescription of a crisis dosage regimen, indicating the need of prescribers training.
