Caracterizaçao da resistencia r oxacilina em estafilococos coagulace negativa isolados no Hospital de Clínicas de Curitiba - Paraná

Estafilococos coagulase-negativa (ECN) são um grupo de microrganismos que constituem parte da microbiota normal da pele, embora seu significado clínico como agente causal em pacientes com materiais implantados como cateter venoso central e periférico, próteses valvulares e outras infecções envolvendo a formação de biofilme tenha aumentado nas últimas décadas. A resistência à oxacilina é uma importante característica deste grupo de bactérias e a dificuldade na detecção fenotípica desta resistência é devido à expressão heterogênea do gene mecA por muitos isolados. Neste estudo, foram avaliados 185 isolados de ECN de hemoculturas colhidas pelo cateter e veia periférica, cultura de ponta de cateter e cultura de válvula cardíaca. Staphylococcus epidermidis foi responsável por 118 episódios (63,8%). A presença do gene mecA foi determinada pelo método da Reação em cadeia da Polimerase (PCR) e 86,5% de todos os isolados foram mecA positivos. A espécie que apresentou maior resistência à oxacilina foi S. haemolyticus (97,91% mecA positivo). O método de disco-difusão mostrou os mesmos resultados para cefoxitina e oxacilina (100% de sensibilidade e 92% de especificidade). A Concentração inibitória mínina (CIM) para a oxacilina pelo método de ágar diluição demonstrou sensibilidade de 100% e especificidade de 84%. Para os dois diferentes testes de triagem avaliados, a sensibilidade e especificidade foram as seguintes: para o teste de triagem com 4 µg/mL de oxacilina, 100% e 92%, respectivamente; para o teste de triagem com 6 µg/mL de oxacilina, 97,5% e 92%, respectivamente. Os métodos fenotípicos que apresentaram melhor acurácia na detecção da resistência à oxacilina e facilidade na execução para laboratórios clínicios de rotina foram disco difusão para oxacilina e cefoxitina e o teste de triagem em ágar com 4 µg/mL de oxacilina. Coagulase-negative staphylococci (CNS) are a group of microrganisms that constitute part of the normal skin flora, although their clinical significance like causal agents in patients with indwelling medical devices such as central and peripheral venous catheters, valvular prostheses and other infections involving biofilm formation on implanted biomaterial have increasing in the last decades. The oxacillin resistance is a important characteristic of this bacterial group and the difficulty in the detection of phenotypic oxacillin resistance is due to the heterogeneous expression of the mecA gene by many strains. In this study, it has been evaluated 185 CNS isolated from blood culture draw through the catheter and peripheral vein, catheter-tip culture and cardiac valve culture. Staphylococcus epidermidis accounted for 118 episodes (63,8%). The presence of mecA gene was determined by Polimerase Chain Reaction (PCR) and 86,5% of all isolates were mecA positive. The specie most resistant to oxacillin isolated was Staphylococcus haemolyticus (97,91% mecA positive). Discdiffusion method showed the same results for oxacillin and cefoxitin (100% of sensibility and 92,0% of specificity). The oxacillin Minimal Inibitory Concentration (MIC) by agar dilution method demonstrated sensibility of 100% and specificity of 84%. For the two different agar screening test evaluated, the sensibility and specificity were as follows: for the agar screening test with 4 µg of oxacillin per mL, 100% and 92,0%, respectively; for the agar screening test with 6 µg of oxacillin per mL, 97,5% and 92,0%, respectively. The phenothypical methods that showed the best acuracy to detect oxacillin resistance and facility to performance in routine clinical laboratories were disc-diffusion to oxacillin and cefoxitin and agar screening with 4 µg of oxacillin per mL.
