dc.description.abstractResumo: Foram estudadas 28 crianças portadoras de cardiopatias congênitas e 3 crianças portadoras de cardiopatias adquiridas, das quais 18 foram submetidas a circulação extracorpórea e 13 não necessitaram deste procedimento durante o ato cirúrgico. Os pacientes foram avaliados durante os períodos pré-operatório, pós-operatório imediato e após 24, 48 e 72 horas do término da cirurgia, procedendo-se às dosagens de água, sódio e potássio intra-eritrocitários e plasmãticos, investigando-se as relações entre as variações desses elementos com a utilização ou não da circulação extracorpórea. Também foram analisados os efeitos do tempo de cirurgia, do tempo de circulação extracorpórea e do balanço de líquidos durante a cirurgia e no período pós-operatório, sobre as dosagens plasmáticas e intra-eritrocitãrias de água, sódio e potássio. Verificou-se que o emprego da circulação extracorpórea não influenciou significativamente sobre a variação dos níveis intra-eritrocitários de água, sódio e potássio; embora os níveis intra-eritrocitários de sódio tenham sofrido elevação no período pós-operatório entre as crianças submetidas a circulação extracorpórea, e redução no grupo de crianças não submetidas a esse procedimento. Os níveis plasmáticos de água aumentaram no período pós-operatório, retornando aos valores basais após 48 horas do término da cirurgia. Houve influência maior entre os pacientes submetidos a circulação extracorpórea, relacionada ao balanço de líquidos deste procedimento. 0 tempo de cirurgia também influenciou significativamente sobre o aumento dos valores plasmáticos de água no período pós-operatório, em ambos os grupos estudados. Os valores pós-operatórios de sódio plasmático sofreram uma redução significativa em relação ao período pré-operatório, independentemente do emprego da circulação extracorpórea. Os pacientes submetidos a circulação extracorpórea apresentaram aumento dos valores de potássio plasmático no período pós-operatório, relacionado ao balanço de líquidos da circulação extracorpórea. 0 conhecimento das alterações no metabolismo intra e extracelular das crianças submetidas a cirurgia cardíaca pode auxiliar no estabelecimento de condições ótimas de hidratação e reposição de eletrõlitos durante os períodos per e pós-operatórios, reduzindo riscos que possam promover o aumento da morbidade e da mortalidade.pt_BR
dc.description.abstractAbstract: Twenty-eight children suffering from congenital cardiopathy and three suffering from acquired cardiopathy were studied. From this total, only eighteen needed extracorporeal circulation during the surgery. The patients were analysed before the surgery, during the immediate post-operative period and also after 24, 48 and 72 hours following the surgery. The levels of intra-erythrocytic and plasmatic water, sodium and potassium were measured and the relationship between the variation of these elements with and without the use of extracorporeal circulation was analysed. The effects of the duration of the surgery, the time of extracorporeal circulation and the balance of liquids during the pre- and post-operative periods on the intra-erythrocytic and plasmatic levels of water, sodium and potassium were also analysed. It was verified that the use of extracorporeal circulation did not have a significant effect on the variation of the intra-erythrocytic level of water, sodium and potassium, although the intra-erythrocytic level of sodium increased during the post-operative period among children who were submitted to extracorporeal circulation and decreased among those who were not. The plasmatic levels of water increased during the post-operative period returning to basal levels 48 hours after the end of the surgery. A more remarkable influence was noticed in patients who were submitted to extracorporeal circulation, which was related to the balance of liquid of such procedure. The time of surgery also had a significant influence on the increase of plasmatic levels of water during the post-operative period in both target groups. The post-operative plasmatic levels of sodium decreased significantly if compared to the pre-operative period, regardless of the use of extracorporeal circulation. Patients submitted to extracorporeal circulation showed an increase in levels of plasmatic potassium after the surgery and this was related to the balance of liquid in such procedure. The knowledge of intra and extracellular metabolic changes in children undergoing heart surgery can help the establishment of optimal conditions of hydration and of replacement of electrolytes during the pre- and postoperative periods, reducing risks of morbidity and mortality.pt_BR
