Entre passado e presente, realidade interna e externa : considerações, a partir de Freud e Winnicott, sobre a primazia do "vivenciar" na clínica

Resumo: O objetivo principal de nosso estudo foi o de refletirmos sobre as vivências na prática clínica psicanalítica, a partir de Freud e Winnicott. Apresentamos como problemática a inserção em uma instituição de saúde pública na qual o adoecimento é compreendido a partir da dimensão orgânica/fisiológica. Visando circunscrever a inserção da psicanálise neste contexto como um campo de vivências específicas, apresentamos então, o modelo onírico em Freud como paradigma do funcionamento psíquico e da prática clínica. Nestas considerações, deparamo-nos com a existência nos sonhos e na transferência de espaços fronteiriços compostos pela sobreposição entre realidade interna e externa, passado e presente e propomos que tais características compõem o âmbito das vivências, distinguindo a prática clínica psicanalítica da dimensão orgânica/fisiológica. Na sequência, abordamos em Winnicott a teoria do desenvolvimento emocional primitivo e o ambiente. Destacamos nestes o brincar e a transicionalidade como sustentação para atualização do modelo de desenvolvimento emocional primitivo na clínica. Neste sentido, apresentamos o campo das vivências como oriundo da identificação entre analista e setting, em referência a identificação mãe e ambiente na dependência absoluta. A partir de Freud e Winnicott, constatamos então que a dimensão das vivências implica na função do analista. No âmbito institucional, este atua como representante da singularidade do paciente no que há de mais particular em cada um: a emergência de conteúdos inconscientes e a possibilidade de experienciar um modo de viver genuíno e criativo. Abstract: The main purpose of this study was to reflect upon the experiences of the psychoanalytic clinic, based on ideas from Freud and Winnicott. The main discussion relays upon the insertion on a public health institution, wherein illness is understood from the physiologic/organic dimension. In order to limit the psychoanalysis inside this context as a field of specific experiences, we exhibit then, the dream model on Freud as a paradigm of the psychic functioning and of the clinical practice. Inside these considerations, we faced the existence in dreams and in the transference of the frontier spaces composed by the overlap between internal and external reality, past and present and we proposed that these characteristics compose the dimension of the experiences, distinguishing the psychoanalytic clinic practice from the physiologic/organic dimension. Then, it is discussed over Winnicott the primitive emotional development theory and the environment. It is highlighted upon these the playing and the transitionality as support for the update of the primitive emotional development model on clinic. In this context, we present the field experiences as coming from the identification between analyst and setting, in reference to identification the mother and the environment in absolute dependence. Based on Freud and Winnicott, we found then that the dimension of the experiences implies on the analyst's function. At the institutional level, it acts as a representative of the uniqueness of the patient in what is most particular in each one: the emersion from unconscious contents and the possibility of genuinely experience an authentic and creative way of living.
